patients check in for appointments at the central reception desk located
in our lobby. We ask that
you bring your insurance card with you to every visit. Co-pays are made at central
reception at the time of check-in and should be made the day of your
appointment. Please notify
us immediately of any changes to your address, phone number or insurance
so that we may serve you more efficiently. If you have changes to be made, please arrive a few
minutes prior to your appointment time.
