Schedule appointments as far in advance as possible.
When requesting
medical records, complete a medical records release form. Your records are confidential and
will not be given to anyone without your written authorization, or as
required by law. We ask for
one week’s notice prior to releasing your medical records.
Feel free to discuss any concerns you may have about our
staff or service with our administrative staff or your doctor. We welcome any suggestions you
might have for improving our services to our patients.
