file a claim to your primary and secondary (if applicable) insurance
carrier when the information has been provided. We do not bill motor vehicle insurance companies, but
we will provide you with an itemized form to submit to them.
Our
primary interest is taking care of your health. If you are concerned about the cost of treatment or
your ability to pay for it, we encourage you to discuss it with our
business office personnel.
